Requirements RN Case Manager at Job Overview Minimum Experience : 1
years in Case Management, specifically in Long-Term Acute Care
(LTAC) or Rehabilitation settings. Experience must include hospital
discharge planning. Rehabilitation hospital experience is preferred
but not required. Skills and Knowledge : Proficiency in Electronic
Medical Record (EMR) systems and documentation per American Case
Management Association (ACMA) Standards of Practice (SOP).
Knowledge of Continuity of Care and Transition of Care requirements
per Centers for Medicare & Medicaid Services (CMS) standards. Shift
: Monday–Friday, 8:00 AM–5:00 PM (5x8-hour day shifts). Any
variation to the 8-hour shift (e.g., staying late or arriving
early) must be discussed and approved by the manager. Work Setting
: Case Management in a 60-bed facility, including a 35-bed
Long-Term Acute Care Hospital (LTACH) unit and a 25-bed Inpatient
Rehabilitation Facility (IRF), with a 20-bed Transitional Care Unit
(TCU) for short-term rehabilitation. Floating : Not specified;
typically limited within the facility due to specialized case
management role. Weekend Requirements : None; shifts are
Monday–Friday. Call Requirements : None. Holiday Expectations : Not
