Job Description:

The Cape & Islands Adult Community Clinical Services (ACCS) operated by the Department of Mental Health (DMH) is seeking an exceptional Licensed Practical Nurse (LPN II) for multiple Seasonal positions to work as a direct service provider while maintaining a person centered, trauma informed atmosphere and professional performance. Knowledge of DMH community residential regulations and program procedures, coordination of all aspects of the program, which includes but is not limited to: monitoring behavioral, psychiatric and health care needs and safety concerns, providing direct nursing and all aspects of care to clients, ensuring that client's needs are met, and overseeing that Community Service Plans are being followed and appropriate documentation occurs, ensure that safe staffing is in place, follow-up of medical appointments, educating clients and staff about rehabilitation, medication and physical and psychiatric illnesses, coordination of physician's orders, prescriptions and recording of all necessary paper work, notification to program director /assistant program director of pertinent information and performs related work as required. This position requires transportation of clients to appointments and other community activities.

Please Note: "This is a Seasonal position for an anticipated duration of 90 days and is 40 hours per week."The selected candidate will work 12:00am to 8:00am. The days off are Tuesday and Wednesday.

Duties and Responsibilities (these duties and responsibilities are not all inclusive):

  • Coordination and monitoring of medication services including obtaining and maintaining current HCP orders, reporting and recording clients' response to medications and ensuring that advocates are reporting symptoms to appropriate care providers
  • Oversees the scheduling of medical appointments and obtaining orders as needed and ensuring documentation and paperwork has been completed.
  • Educating staff and clients about medical conditions, medications and their effects by using professional knowledge and resources available in addition to conducting Medication Teaching for assigned individuals.
  • Assist clients in a teaching atmosphere to educate them in Assisted Daily Living (ADL) skills such as personal hygiene, cooking, housekeeping, clothing maintenance and nutrition by utilizing behavior modification techniques, counseling techniques, role modeling and other individual or group methods to promote client grown.
  • Documentation of the behavioral and medical observations and individual's progress with issues addressed in their Individualized Action Plan (IAP).
  • Ability to apply psychiatric nursing care when interacting therapeutically with individuals served while assessing needs, discussing care, allaying fears and anxieties to provide a therapeutic environment.
  • Demonstrated talent for providing positive peer interaction and support along with initiative and independence with tasks not assigned.

Required Qualifications:

  • Working knowledge of the established nursing procedures for the administration of medications.
  • Ability to read, write, speak, and comprehend English, to communicate effectively with a diverse group of patients, employees, and supervisors.

Preferred Qualifications:

  • Experience working with individuals with serious, persistent mental illness, substance abuse and/or who have experienced homelessness.
  • Given the population served, bilingual or multilingual fluency.

Special Requirements:

  • Possession of a current and valid Massachusetts Class D Motor Vehicle Operator's license or equivalent is required for this function.
