Continuum, is deeply passionate about understanding what matters to
people to ensure the delivery of Real-Person Care. We create an

emotional health. Our Care Solutions, Analytics, Implementations,
Clinical and Business Operations, and Learning and Development
teams work tirelessly in partnership with our clients and
stakeholders in communities to address the real-life healthcare
needs people have every day. This role is based in the Boston
region, working with Tufts Medicine. As the Director, Post-Acute
(Tufts), you will: Implement and monitor data driven performance
dashboards for Post-Acute Alliance reporting. Regularly review, and
report monthly, on the performance of Post-Acute Care Alliance
providers through quarterly performance scorecards, ACO expenditure
reports, claims data, and other reporting to drive performance
within value-based payment models and risk-based contracts.
Identify performance gaps, highlight trends, and develop corrective
action strategies to progress toward goals. Provide regular
report-outs to internal and external teams to allow visibility into
post-acute performance. Meet routinely with post-acute providers to
enhance performance and facilitate the exchange of best practices.
Conduct sessions with each skilled nursing facility in the PAC
alliance to maintain open lines of communication, confirm progress
toward shared goals, pinpoint areas for improvement, and create
action plans to support goal achievement. Work across skilled
nursing facility (SNF), home health partners, and internal
stakeholders to advance clinical capabilities in the SNF Alliance,
to care for an increasingly complex patient mix. Lead a team of
Post-Acute Care associates in delivering high-performance case
management by improving proactive discharge planning timeliness
ensuring compliance with care coordination standards across all
skilled nursing facilities and post-acute care programs. Ensure
collaboration between PAC associates and key client partners
—including Care at Home (Home Health), hospital care management,
ambulatory care management, value-based care program leads, and
health plans—to streamline processes, efficiently manage patients,
and provide a coordinated approach to care delivery and outcome
optimization. Review performance metrics of non-alliance
agencies/facilities evaluating performance opportunities. Such
review may include CMS care compare ratings, referral acceptance
rates, denials, and readmissions. Collaborate with the market
leadership and other members of the executive and enterprise
leadership team to achieve outcome measures through efficient
communication across the care continuum. Present and communicate
post-acute strategy and performance to fellow leaders and
executives. Other duties as required. As a Director, Post Acute you
will: As a member of the Navvis Boston market Post Acute team, you
will play a key role in developing collaborative workflow standards
to enhance care continuity, improve quality, and support the
efficient use of resources for patients following discharge through
ongoing, data-informed performance analysis. Work collaboratively
with aligned Post-Acute Care providers , the In-Market One Care
Team, Care at Home (Home Health), and other entities as necessary
to drive performance through quarterly scorecard review sessions,
problem solving, referral processes, ongoing monitoring and action
planning. The Director will oversee the PAC Associates to guarantee
achievement of day-to-day operational goals and foster clear
communication, supporting ongoing performance improvement and
cohesive care initiatives. Strengthening Alliance performance by
implementing quarterly improvement plans and achieving key metric
goals such as referral acceptance rates, length of stay efficiency,
and readmission reduction. Integrate new strategic initiatives—such
as care pathway redesign, SNF clinical capability expansion, and
One Care Team alignment—ensuring Alliance partners adopt updated
workflows within 90 days of rollout. Develop and maintain
collaborative relationships with internal and external
stakeholders, achieving measurable year over year improvement in
shared key performance indicators defined across the Alliance, Care
at Home, One Care Team partners, and Care Continuum leadership.
Continuously assess regulatory and market challenges and translate
findings into actionable strategies to improve quality and
financial performance. Support and lead Alliance development
initiatives, including evaluating network composition annually and
executing measurable changes (e.g., onboarding, performance
remediation, or removal) aligned with established performance
thresholds. What Success Looks Like in this Role: Strengthening
Alliance performance by implementing quarterly improvement plans
and achieving key metric goals such as referral acceptance rates,
length of stay efficiency, and readmission reduction. Integrate new
strategic initiatives—such as care pathway redesign, SNF clinical
capability expansion, and One Care Team alignment—ensuring Alliance

thresholds. Requirements We are excited about you if you have these
things: Possess a valid clinical licensure for Massachusetts in one
of these disciplines RN, SW, PT, or OT. 5 years of strategy,
operational, or clinical leadership in the post-acute industry
Experience in operations or leadership within Skilled Nursing
Facilities, Inpatient Rehabilitation, Home Health, and/or Hospice
Proven experience in creating and executing data-driven performance
improvements An understanding of healthcare economics and
regulatory requirements governing skilled nursing/home health, BPCI
A, MSSP/ACOs, CMS Waiver Programs, etc. Organizational skills, with
an ability to prioritize multiple requests and deadlines Strong
leadership, presentation, and communications skills to achieve
buy-in and collaboration toward objectives Experience with
Microsoft Office, Word, PowerPoint, Excel and Teams, plus other
virtual conferencing platforms Must be in the Boston, MA area
(Hybrid-Remote/In-Market Boston, MA) Travels up to 80% to
