Summary: We are seeking a compassionate and reliable Licensed
Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) to provide
high-quality nursing care to patients in their homes. The Home
Health LVN/LPN delivers skilled nursing services under the
supervision of a Registered Nurse or physician, supporting patient
recovery and independence in a home-based setting. Qualifications:
Graduate of an accredited LVN or LPN program Active LVN/LPN license
in the state of Massachusetts 1 full year Adult Home Health
experience. BLS certification required Valid driver's license and
reliable transportation Skills & Competencies: Strong clinical and
patient assessment skills Excellent communication and time
management abilities Ability to work independently in home-based
environments Compassionate, patient-centered approach to care
Strong documentation and organizational skills Physical
Requirements: Ability to stand, walk, bend, and lift as required
Ability to assist patients with mobility and positioning Ability to
travel between patient locations Key Responsibilities: Provide
skilled nursing care to home health patients per plan of care
Monitor and document patient conditions, vital signs, and treatment
responses Administer medications and treatments as prescribed
Perform wound care, catheter care, and basic clinical procedures
Educate patients and families on care plans, medications, and
safety Communicate patient status changes to supervising RN or
physician Maintain accurate and timely documentation in the EMR
Adhere to infection control, safety, and home health regulations
Travel to patient homes within assigned service area
