This one-on-one healthcare approach allows you to connect and collaborate with individuals on their care, while acting as an advocate for their needs and quality of life. Responsibilities include: /n/n - Conducting comprehensive assessments of patients' health status and developing individualized care plans in collaboration with the interdisciplinary team /n/n - Providing skilled nursing care to patients in their homes, including medication management, wound care, and infusion therapy /n/n/n - Educating patients and their families on disease management, medication administration, and other aspects of health to promote self-care and independence /n/n - Monitoring patients' progress and response to treatment, adjusting care plans as needed to achieve desired outcomes /n/n - Providing emotional support and counseling to patients and families coping with chronic illness or end-of-life care /n/n /n/n In the Hospice setting, you will be a vital link between a patient and their family to resources across the care spectrum.
