Job Title: Home Health RN Profession: RN Home Health Specialty: RN Home Health Duration: 13 weeks Shift: 08:00 AM - 04:30 PM Hours per Shift: 8 Experience: Minimum 1 year of Certified Home Care experience and 1 year of Case Management experience License: Current RN license Certifications: None specified Must-Have: Ability to travel to assigned cases; Weekend rotations every 6 weeks Description: All applicants must have 1 year of Certified Home Care experience. All applicants must have 1 year of Case Management experience. Candidates should have the ability to travel to cases as assigned. Weekend rotations are required once every 6 weeks. As a Home Health RN, responsibilities include assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families. Patient care is delivered within organizational philosophy, policy, and standards of community health practice. This role also requires performing admission processes and case management responsibilities in accordance with organizational policies and standards of practice. Utilization of thorough and timely electronic documentation and interdisciplinary communications is essential.
