RN Case Manager: The Clinical Case Manager will work with an
interdisciplinary team ensuring continuity of care, managing and
modifying treatment plans, discharge planning, reviewing and
analyzing case management reports, etc. * Must be a Registered
Nurse with active MA RN license * Prefer 2 years related clinical
experience * Certified Case Manager (CCM) preferred * BLS Certified
