We are neighbors helping neighbors.We are looking for an experienced, talented and compassionate RN Baylor for a full-time position located in the South Shore area.What will you be doing:The Hospice RN Baylor will develop, implement and evaluate outcome-oriented patient care plans requiring specialized nursing skills. In this role, you will obtain family and or patient involvement in the development of such plan whenever possible; complete all necessary documentation, according to Agency policies and procedures. You will also conduct initial evaluation visits and regularly reevaluate the nursing needs of the clients; initiate, develop, implement, and make necessary changes to the clients plan of care. It is crucial for the Hospice RN Baylor to assess client's continual care needs and informs physician and any other appropriate health professionals, as often as needed, of any changes in the client's conditions.
